Before Conference

APPLICABLE TO ALL SPEAKERS:

Please include 25% of your time for discussion/throughout the presentation. Please be ready with discussion questions to pose to the group to help facilitate discussion if needed. This is required for the conference in order to meet accreditation standards.

COMPLETE ALL SESSION REQUIREMENTS (BY JANUARY 20, 2017)

Tip: If you are speaking in multiple types of presentations, please take careful note of the parts in blue so you do not submit redundant information.

A. AV (Audio-Visual) Requirements (one per presentation). Please verify and confirm if you need additional equipment to what is already provided (eg. flip chart with markers, WiFi connection, etc) by submitting the AV form.

B. Disclosure of Conflict of Interest (one per person). Speakers must fully disclose any affiliation (financial or otherwise) with a commercial or other industry interest. Please complete the "Disclosure of Conflict of Interest" form and return it to our office by January 20, 2017.

Conference Syllabus Materials (one per presentation). We will be preparing a syllabus and must receive content materials for your presentation(s).  Please submit your materials in Word or PowerPoint format only, we are not able to accept PDF submissions.
